I too am having this error. I have to do a service fop2 restart to clear it.
Had one of our users have "Flash Player Required" pop up this morning.
I verified flash is installed and working.
XP 32bit, IE 8.0.6001.18702, Flash 11.1.102.62
FOP2 2.24
I cleared cache.
Tried uninstalling flash and reinstalling.

Was trying to 'listen' in on an extension and discovered an interesting bug.

Call out via iax trunk. Call was muted on our end.
I used fop2 to 'listen' in on said call by highlighting the extension then clicking the ear.
I couldn't hear anything. When I hung up my end, fop2 showed I still had a line in use. SIP Show Channelstats confirmed the same. Tried listening in again to see if that would clear it but it only added another channel.
Channel request hangup did not work.
Un-muting the main call didn't affect the orphaned channels.
Hanging up the main call did result in the channels being hungup/released.
Reproducible.

This is a licence we got in 2009. We upgraded/updated in May 11 at which time we bought a license upgrade. You had to reset something at that time I believe to get it going.
I usually chat with you on gmail, so I'll keep an eye out when you hop online again.

I built up a new box with ast10 and just put it online.
Went to revoke on the old box and got "Serial code not available for revokation on this machine!"
The code I'm using is the upgrade code we bought in May of last year.
Same version of fop2 on both machines.

Thanks to Nicolas's help, a simple emptying of the browser cache fixed my problem.
I've upgraded to 2.24 but am having issues. At first, Chrome would connect but there would be a gray box at the bottom of the screen with what looks like some code in it. But everything else was functional.
Was unable to access the phonebook, so I went into the fop2 freepbx module and did a fop2 permissions update (hadn't don't before, did it individually in the users module before). Now, unable to connect to the panel using chrome at all. It is at the "One Moment Please" screen with the little circle rotating. No attempts.
I did a service fop2 restart, but that didn't work either.
IE8 works fine.
Issue ended up being with FOP1 interfering. I have it disabled now and things look to be working normally.

My XP virtual machine had a firefox update this morning and is now exhibiting the same issue. I'll do a capture as suggested.
Mine is instantaneous. That is, I can start up the panel, see a current phone call yet if that phone call hangs up, it doesn't show on the fop2 panel. It still shows connected until I do a page refresh.
I keep an XP virtual open all day anyways and I'm the only one using Ubuntu, so I'll just live with it. Though it would be nice if it could be addressed.
