I've just added a 14th button (a new trunk), but can't get it to show up. So I deleted an unused extension, and now the trunk shows up.
Maybe there's an off-by-one error somewhere?

I can't find where slider.js and sound.js are mentioned.

It seemed like this could be fixed in the sql in the autoconfig-buttons-freepbx.sh (by only selecting trunks that aren't disabled), but if you have the freepbx module installed, it seems that the buttons' existence is created by the module, not the autoconfig...
2. How can we remove a button from displaying. In essence we do not wish to display the default zap/g0 trunk as it will not be used.
Me too. I have two trunks that I want to keep around, but aren't presently using. Merely disabling them doesn't remove them from the Trunks buttons. Maybe a disabled trunk shouldn't have a button at all? Maybe there should be a check box "hide" in the freepbx module somewhere (per trunk, or per button)?
fop 2.1 out of the box recognized the custom trunk. The module let me rename to be more sensible -- very easily. So far, terrific!
How do I get the latest version?
I have one trunk set up for outgoing toll-free calls, so that they calls go to a free 800-number trunk and don't take up any of my paid trunks. However, the way that's set up, as a 'Custom Trunk'....
... and it doesn't show up under Trunks in fop2.
Is this a button configuration problem (auto config doesn't get custom trunks?)? Or something else?

It's a browser/plugin issue. It might work if you reinstall the adobe flash player. Also, be sure to disable any plugins that prevents flash from working, or add the proper exceptions to them.
Yep, yep. I use "Flashback" (which replaces flash content with a button to activate it). Luckily, it has a whitelist feature, so I just whitelisted my Asterisk box and all's well.

Do you have adobe flash player installer? You might also want to try with another browser or from another workstation. If you connect from the internet/remotely, check you do not have a firewall blocking access to port 4445.
Son of a gun. Doesn't work on Firefox 3.6.2, works fine on IE 8.0.7600.16385. Strange. Firefox has version "WIN 10,0,42,34" (according to http://kb2.adobe.com/cps/155/tn_15507.html).
